Councillor Applauds Decision of Capital Health to Deliver Services in East Dartmouth

(October 23, 2008) Councillor Andrew Younger today applauded a decision by Capital Health's site selection team to locate a Community Health Team in East Dartmouth to further the services being developed in conjunction with the new East Dartmouth Community Centre.

Councillor Younger was part of a group which included the local Community Health Board, Boys and Girls Club of Dartmouth East, and area residents lobbying for the delivery of community health services in the area.

"The work of the Community Health Team in East Dartmouth will improve opportunities for area residents and will fill an important gap in service delivery options in the area," says Councillor Younger. "I am pleased to have been part of the team working to secure these much needed services."

The Community Health Team is a new community-based health model that focuses on chronic disease prevention and management.

Community Health Teams will support individuals and families to build knowledge, confidence and skills to better prevent and manage chronic disease. Community Health Teams will be led by Primary Health Care Capital Health; in partnership with the IWK Health Centre, and will collaborate with individuals and groups across the continuum of care, as well as with community partners, throughout development and implementation. Communities will be active participants throughout the process, identifying needs and influencing the programs, services and supports that make up the Community Health Team.
